Home and Community-Based services are provided to at-risk children, adolescents and families with a goal to provide stabilization within the community and/or home environment. Hill Valley home-based counselors utilize community resources and collaborate with community professionals to meet individual/family treatment goals, thus preventing out-of-home placement.
